Incident Reporting and Just Culture

By VeriScorePrep Team ยท 4 min

An incident report is an internal document recording an event that affected or could have affected patient safety. It is not part of the medical record and does not serve blame; it feeds a learning system.

What triggers a report

A medication error, fall, equipment failure, patient or staff injury, near miss, complaint, or security event. Near misses are reported too โ€” they reveal system weaknesses before harm occurs.

The report

Document facts objectively: what happened, when, who was involved, what was done immediately, and what follow-up occurred. Do not include opinions, accusations, or assign blame. Use quotes for statements. The report goes to risk management or the designated safety office.

The medical record

Document clinically relevant facts in the patient's record โ€” the patient's status, your assessment, the intervention, and the plan. Do not mention "incident report" in the medical record; that document is separate.

Just culture

A just-culture approach distinguishes human error (manageable, system-level fix), at-risk behavior (coach, redesign incentives), and reckless behavior (rare; disciplinary). The goal is a system that surfaces error without driving it underground.
